<p><span style="color: #333333; letter-spacing: 0.208px;">But in August 2015, before she made a gastroenterology appointment, Wells experienced an unnerving episode. Her left leg and lower lip suddenly went numb, and her tongue began tingling. Wells said she didn’t think she was having a stroke because she could think clearly; the symptoms abated within hours. She saw a neurologist, who suspected multiple sclerosis or a vitamin B deficiency, both of which were soon ruled out. But a nerve conduction test, which uses electrodes attached to the skin to assess damage, showed decreased rates of nerve conduction in her left leg and both feet.</span></p>
<p><span style="color: #333333; letter-spacing: 0.208px;">Because no underlying cause could be found, Wells was diagnosed with idiopathic degenerative neuropathy — nerve deterioration for no apparent reason — and advised to stay physically active to preserve muscle function.</span></p>

<title>Making it Real: SNPs, Nutrient Deficiency, Total Body Burden, and the Manifestation of Disease</title>

<span style="color: #000000;">We received several comments/questions after Steve Ottersberg’s lecture on how to apply the principles of snps and detoxification in patient care. So I thought I would fill in a few blanks.<br />
</span></p>
<p><span style="color: #000000;">First here is a recent article by Jeffrey Bland and Deanna Minich outlining the rationale and some of the data behind the efficacy of personalized medicine: <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3710624/pdf/TSWJ2013-129841.pdf" target="_blank">Personalized Lifestyle Medicine.</a>
</span></p>
<p><span style="color: #000000;">And another short but relevant article on the topic of <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4170107/" target="_blank">Detoxigenetics: In Search of Host Susceptibility to Environmental Toxicants.</a> </span></p>
<p><span style="color: #000000;">I’ll come back to this last article in a few minutes so you might want to read it before moving on here.</span></p>
<p><span style="color: #000000;">The $99.00 panel for saliva-derived gene polymorphisms that are offered direct to patients by 23andme have some drawbacks (contamination with bacteria that may have sulfite oxidase (SUOX) snps) but the affordability of the test may make up for the reliability of that specific snp. The Illumina chip that they use tests for over 200,000 snps plus 30,000 they have identified from the medical literature that correlate with disease states. Blood testing with Amy Yasko PhD, who runs and interprets personalized snp profiles for autistic children, charges $500.00 for the panels of specific snps she has found to be helpful in this population (no affiliation although I do think she deserves attention).</span></p>
<p><span style="color: #000000;">Personalized lifestyle medicine, as it’s being called, is becoming more and more popular in medical and academic circles, particularly with current research&nbsp;on MTHFR variants and remission of depression in those who had previously failed SSRI therapy.</span></p>
<p><span style="color: #000000;">Fortunately, you don’t need to have the methylation snps, their effects, or interventions memorized. Dr Jim Roberts, a nutrition-oriented cardiologist has utilized the work of Amy Yasko &nbsp;and created a <a href="http://www.heartfixer.com/AMRI-Nutrigenomics.htm" target="_blank">page on his practice website on Nutrigenomics</a> that, in great detail, explains all of the methylation snps, their place in the cycle, and suggested nutrient supplementation for each.</span></p>
<p><span style="color: #000000;">What is less well-recognized, evaluated or utilized in integrative medical practice, but equally important, is the role of snps in “detoxification” or as we like to correctly term it: biotransformation. Let’s take a look at some 23andme results:</span></p>
<p><span style="color: #000000;">Here are the results of one 23andme salivary gene array testing that is then put through the no-cost analysis at www.geneticgenie.org to produce two panels: Methylation Profile</span></p>

<p><span style="color: #000000;">For now, I’m just going to address the Biotransformation snps.</span></p>
<p><span style="color: #000000;">Dr. Merritt will talk more about methylation snps and lead toxicity/chelation in her upcoming lecture.</span></p>
<p><span style="color: #000000;">And now, about that toxicogenetics article I linked above:</span></p>
<p><span style="color: #000000;">It has been reported that organochlorine insecticides are present at a higher concentration in tissues of Parkinson’s Disease patients, which may partly explain the association between PD and polymorphisms within the&nbsp;CYP2D6&nbsp;gene. <a href="http://journals.plos.org/plosone/article?id=10.1371/journal.pone.0084413" target="_blank">A recent meta-analysis</a> did show a significant increased risk for PD in those with the CYP2D6*4 snp.</span></p>
<p><span style="color: #000000;">This relationship <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4170107/" target="_blank">includes risk for Parkinson’s Disease after exposure to paraquat</a>&nbsp;a combination that gives an 11 times increased odds ratio to those in the study with a CYP2D6*4 snp. Why? Because CYP2D6 is the pathway for biotransformation and elimination of most herbicides and pesticides as well as about 25% of pharmaceuticals.</span></p>
<p><span style="color: #000000;">Individuals with the CYP2D6*4 &nbsp;mutation, the most common CYP2S6 snp, produces non- functioning 2D6 enzymes. What does this mean? From the study linked above: “About 5–10% of white populations are homozygous for the allele and for them the enzyme activity is practically undetectable” Slowly metabolized pesticides and herbicides have increased carcinogenicity and neurotoxicity.&nbsp;&nbsp;The panel above does not show evidence of CYP2D6 snps but if it did and that individual had a occupational history of organochlorine exposure, especially if they exhibiting early signs of PD (anosmia, sleep disturbance, tremor, soft voice, episodic dizziness, etc.) -an appropriate intervention may be not only to initiate a process of decreasing body burden (Dr. Genuis will lecture on this January 5th) through medical sauna, but also initiate a protocol to decrease the neurotoxicity of organochlorine pesticides via safe pancreatic-lipase inhibitors. Read <a href="https://www.hindawi.com/journals/tswj/2013/205621/" target="_blank">Dr. Genuis’</a> article on this subject for a thorough education on reducing organochlorine pesticide body burden.</span></p>
<p><span style="color: #000000;">Finally, this panel also shows a null GSTT1. GSTT1 deletions are related to development of <a href="https://oup.silverchair-cdn.com/oup/backfile/Content_public/Journal/carcin/31/2/10.1093_carcin_bgp286/2/bgp286.pdf?Expires=1492281277&amp;Signature=M61-bhwOZkCjfGjlvFqnMybtwWTYMKx9I5xY6Ys00AujvRJjPxEuGpVU5aCGwrtrJ75mMdlaFh5TWZe9NahduuV2NYxUhf~kJjl6ntBLwZhdkWVv9WWlwrGWjn9WX-V-oSlzh0YHxwyxujk6ViiqjVE1b0sHAFdb4vloCwdgGIU2-3pa3MW-CeciPAKFMPYH5MMS6LmPPJCW5-kIXqYmPYUywZvfVP3iTu~YlLZCqALLwvydDMt7SpjstvNaBa~d77tlx~eZJI45OTkWK9u8rYjm8-bEu91qtKIlyUAL18XNWMlOriNZf7R2MYJl84ReqAO4wr~xvSPO8CjAvC-Flg__&amp;Key-Pair-Id=APKAIUCZBIA4LVPAVW3Q" target="_blank">colorectal</a> and <a href="https://www.ncbi.nlm.nih.gov/pubmed/23637998" target="_blank">lung cancer</a> as well as <a href="http://cebp.aacrjournals.org/content/9/1/73.long" target="_blank">gastric cancer</a> in a Chinese population&nbsp;and invasive <a href="https://link.springer.com/article/10.1007%252Fs12156-008-0054-4" target="_blank">ductal breast cancer in women</a> with both the GSTM1 and GSTT1 mutation. Glutathione S-transferase &nbsp;(GST) enzymes are found in the liver, gut, brain and other tissues and are involved in detoxification of a variety of endogenous and exogenous substances and protect tissues against oxidant stress. The two snps of the GSTT and GSTM variants (GSTT1*0 and GSTM1*0) are deletions, meaning that variant of the enzyme has no activity. Approximately 50 and 20% of Caucasians are homozygous for the GSTM1∗0 and GSTT1∗0 genotypes, respectively. <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3572192/" target="_blank">A study</a> of GSTT1 homozygous mutation holders that had been exposed to paraquat were shown to be at 11 times increased risk of PD compared to those without the mutation who WERE exposed to paraquat.</span></p>
<p><span style="color: #000000;">Unfortunately our patient whose panel is listed above DOES have a GSTT1 deletion and DOES have an occupational exposure to paraquat (one of the most widely used herbicides in the world) and does have early symptoms of PD. What is our intelligent intervention for her? I’ll talk about GSTT1 and GSTM1 deletions at length next week, until then, let me know what YOU suggest. Remember this is not a glutathione deficiency, it’s a gene mutation that prevents glutathione from forming a conjugate with a toxic molecule. How can we intervene and decrease risk from toxicant-induced disease? Does supplementing with glutathione precursors (ALA, NAC, etc.) improve the functional effect of GST enzymes? Under a court order, last week California public health officials released draft <a href="http://www.sfchronicle.com/file/198/6/1986-Cell%2520Phones%25201-26-15.pdf" target="_blank"><strong>guidelines</strong></a> advising cell phone users to keep phones away from their bodies, use speaker phone and limit use. The guidelines, first reported by the San Francisco Chronicle, advise Californians that scientific studies have linked electromagnetic radiation from long-term cell phone use to an increased risk of brain cancer and other health problems.</span></span></span></p>

<p>At a May 9 seminar, a trio of researchers highlighted how environmental medicine can help reduce leading health problems, including diabetes, obesity, and other chronic diseases.<br>
<br>
“Obesity, Electrosensitivity, Mycotoxicosis, and the Clinical Practice of Medicine,” hosted by the NIEHS Office of the Director, focused on the role of chemicals, mold, food, electromagnetic energy, and other environmental factors in making people sick, and how removing triggers from a person’s environment can improve health. This patient-centered, cause-oriented model for diagnosing, treating, and preventing chronic diseases is known as environmental medicine.<br>
<br>
Lisa Nagy, MD, director of the Environmental Health Center of Martha's Vineyard, shared her personal story of surviving multiple chemical sensitivities and discussed her work teaching, practicing, and advocating for environmental medicine. Magda Havas, PhD, from Trent University in Ontario, and William Meggs, MD, PhD, from East Carolina University, presented specific examples from their own practices.<br>

<p>Even if you're not in Flint, Mich., there are toxic chemicals in your home. For that matter, in you.&nbsp;<br>

Scientists have identified more than 200 industrial chemicals — from pesticides, flame retardants, jet fuel — as well as neurotoxins like lead <a href="http://www.scientificamerican.com/article/newborn-babies-chemicals-exposure-bpa/" target="_blank">in the blood</a> or <a href="http://www.npr.org/2012/05/16/152818798/breasts-bigger-and-more-vulnerable-to-toxins" target="_blank">breast milk </a>of Americans, indeed, in people all over our planet.<br>

These have been linked to cancer, genital deformities, lower sperm count, obesity and diminished I.Q. <p>The <a href="http://abcnews.go.com/topics/news/us/environmental-protection-agency.htm" target="_blank">Environmental Protection Agency</a> says it is now investigating a massive ongoing natural gas leak in Los Angeles' Porter Ranch neighborhood that's being called one of the worst environmental disasters in years.<br>
<br>
More than two months after the leak started, methane gas continues to escape from an underground pipe at Southern California Gas Company's Aliso Canyon facility. SoCal Gas acknowledged early in December that a leak had been detected at a natural gas storage well on Oct. 23, but said "experts agree that natural gas is not toxic."<br>
<br>
Thousands of residents, however, said the gas smell -- stemming from an odorous additive called mercaptan used to signal a leak -- made them feel sick, bringing on headaches, dizziness and nausea.<br>
<br>
"We're devastated," resident David Balen told <a href="http://abc7.com/news/gas-company-finds-location-of-leaking-porter-ranch-well/1138585/" target="_blank">KABC-TV</a>. "Our community has been shaken up. Our schools have been shaken up. People have been relocated.”<br>
<br>
According to KABC-TV, SoCal Gas had helped place more than 2,200 families. The situation even drew the attention of celebrity Leonardo DiCaprio, an environmental campaigner. Methane can contribute to climate change.

<p>FLINT, MI — You may have thought Flint's lead in water crisis was drawing to a close, but the Hurley Medical Center doctor who <a href="http://www.mlive.com/news/flint/index.ssf/2015/09/study_shows_twice_as_many_flin.html" target="_blank">played a critical role</a> in ending the city's use of the Flint River has other plans.<br>
<br>
"I am not going to sit back and just see what happens ... so that in 20 years these Flint kids will (have been) nothing more than statistics on the consequences of lead poisoning," said Dr. Mona Hanna-Attisha.<br>
<br>
"We can sit back and do nothing, but we have an incredible opportunity to do something," she said.<br>
<br>
The director of Hurley Medical Center's Pediatric Residency Program, whose research first showed rising levels of lead in the blood of Flint children, is expanding her data-driven work and also pushing to create a model public health program here — one aimed at offsetting the damage that's been caused by high levels of lead in water. <br>
<br>
A mother of two who learned pediatrics as a medical student in Flint 15 years ago, Hanna-Attisha went public with her research on blood lead levels in infants and children on Sept. 24, a time when city officials were still insisting that the city's drinking water was safe and state government officials initially questioned the validity of her work.<br>
<br>
One day after the release of the study, former mayor Dayne Walling, who pushed the ceremonial button to change the city's water source from Detroit to the Flint River, joined in declaring a public health emergency in response to spiking lead in water levels.

<p>The short answer is yes. While the Environmental Protection Agency regulates municipal tap water and <a href="http://www.epa.gov/laws-regulations/summary-safe-drinking-water-act" target="_blank">sets legal limits on certain contaminants</a>, and most water utilities generally stay within these limits, “some of the legal limits may be too lenient,” said Paul Pestano, a research analyst with the Environmental Working Group. And more than half of the chemicals found in municipal water are not regulated.<br>
<br>
Using the <a href="http://www.ewg.org/tap-water/" target="_blank">right water filter</a> can help further reduce pollutants like lead from old water pipes, pesticide runoff in rural areas and byproducts of chemicals like chlorine that are used to treat drinking water. Radon, arsenic and nitrates are common pollutants in drinking water, and trace amounts of drugs including antibiotics and hormones have also been found. Certain filters may help remove these impurities as well.</p>

<p>Exposure to toxic environmental chemicals during pregnancy and breastfeeding is ubiquitous and is a threat to
healthy human reproduction. There are tens of thousands of chemicals in global commerce, and even small exposures
to toxic chemicals during pregnancy can trigger adverse health consequences. Exposure to toxic environmental
chemicals and related health outcomes are inequitably distributed within and between countries;
universally, the consequences of exposure are disproportionately borne by people with low incomes. Discrimination,
other social factors, economic factors, and occupation impact risk of exposure and harm. Documented links
between prenatal exposure to environmental chemicals and adverse health outcomes span the life course and
include impacts on fertility and pregnancy, neurodevelopment, and cancer. The global health and economic burden
related to toxic environmental chemicals is in excess of millions of deaths and billions of dollars every year.
On the basis of accumulating robust evidence of exposures and adverse health impacts related to toxic environmental
chemicals, the International Federation of Gynecology and Obstetrics (FIGO) joins other leading reproductive
health professional societies in calling for timely action to prevent harm. FIGO recommends that
reproductive and other health professionals advocate for policies to prevent exposure to toxic environmental
chemicals, work to ensure a healthy food system for all, make environmental health part of health care, and
champion environmental justice.<br>

<p>Breastfeeding appears to expose infants to a group of industrial chemicals linked to immune system problems, according to a study released today.
The study is the first to estimate the transfer of water- and stain-proofing chemicals from mother to baby during breastfeeding and suggests that the mother’s milk—which provides healthy antibodies, vitamins and nutrients— is also a major source of these harmful compounds for the developing children.<br>
<br>
“It’s a very unwelcome surprise that these compounds that we’re just beginning to understand to be toxic actually transfer through human milk to the most vulnerable population group,” said Philippe Grandjean, a professor of environmental health at the Harvard T.H. Chan School of Public Health and co author of the study published today in Environmental Science and Technology.</p>

<p>Arsenic has two chemical forms, inorganic and organic (the latter of which can be less toxic), and is naturally part of the minerals in the earth's crust. (Note, here organic is a chemistry term and should not be confused with food sold as “organic.”)  Arsenic also has been released into the environment through the use of pesticides and poultry fertilizer. (Chickens can be fed arsenic.) Therefore, it’s in soil and water. Rice tends to absorb arsenic more readily than many other plants.<br>
<br>
Regular exposure to small amounts of arsenic can increase the risk of bladder, lung, and skin cancer, as well as heart disease and type 2 diabetes. Recent studies also suggest that arsenic exposure in utero may have effects on the baby’s immune system.</p>

For years, public campaigns have been waged against plastic containing bisphenol-A (BPA), a controversial plastic additive, due to concerns about adverse human health effects caused by the exposure to synthetic estrogen. But a new investigation by Mother Jones reporter Mariah Blake has revealed that chemicals used to replace BPA may be just as dangerous to your health, if not more.<br>

Several chemicals including polychlorinated biphenyls are metabolized by the dioxin-inducible enzyme CYP1A1; some of their metabolites can interact with the TH receptor. In animals, this mechanism is reflected by a strong correlation between the expression of CYP1A1 mRNA and TH-regulated mRNAs. If this mechanism occurs in humans, we expect that CYP1A1 expression will be positively correlated with the expression of genes regulated by TH. Objective: The objective of the study was to test the hypothesis that CYP1A1 mRNA expression is correlated with TH-regulated mRNAs in human placenta. Methods: One hundred sixty-four placental samples from pregnancies with no thyroid disease were obtained from the GESTE study (Sherbrooke, Qéubec, Canada). Maternal and cord blood TH levels were measured at birth. The mRNA levels of CYP1A1 and placental TH receptor targets [placental lactogen (PL) and GH-V] were quantitated by quantitative PCR. Results: CYP1A1 mRNA abundance varied 5-fold across 132 placental samples that had detectable CYP1A1 mRNA. CYP1A1 mRNA was positively correlated with PL (r = 0.64; P &lt; .0001) and GH-V (P &lt; .0001, r = 0.62) mRNA. PL and GH-V mRNA were correlated with each other (r = 0.95; P &lt; .0001), suggesting a common activator. The mRNAs not regulated by TH were not correlated with CYP1A1 expression.
